Title
A MOTION authorizing King County to accept a donation by Pfizer Patient Assistance Foundation of pharmaceutical products through its Sharing the Care Product Replenishment Program to eligible patients of the department of public health and authorizing the executive to enter into an agreement with the foundation that describes the terms and procedural requirements issued by the foundation for participation in the program.
Body
      WHEREAS, Pfizer Patient Assistance Foundation, through its Sharing the Care Product Replenishment Program, provides certain Pfizer products to eligible community health centers for eligible patients, and
      WHEREAS, the Seattle-King County department of public health is an eligible entity under the program and desires to participate;
      NOW, THEREFORE, BE IT MOVED by the Council of King County:
      A.  King County hereby accepts the donation by Pfizer Patient Assistance Foundation of pharmaceutical products for eligible patients of the Seattle-King County department of public health, the annual value of which is estimated to be $35,000.
      B.  The executive is authorized to enter into an agreement, substantially in the form of Attachment A to this motion, with the foundation that describes the terms and procedural requirements of the program.
