Title
A MOTION requesting the executive, in collaboration with the departments of public health and community and human services, and a community stakeholder panel informed by local and national expertise, to develop and submit for council review and approval a plan for an accountable and integrated system of health, human services and community-based prevention in King County.
Body
      WHEREAS, King County has an adopted strategic plan that includes the goal to provide opportunities for all communities and individuals to realize their full potential, and
      WHEREAS, King County established and adopted the framework policies for human services 1999 and revised them in 2007, and continues to use the framework policies to guide the county's investments in human services and communicate King County's role in human services, and
      WHEREAS, in 2007, King County established and adopted the public health operational master plan to guide broad public health policy and decision-making related to the delivery of public health services in King County, and
      WHEREAS, King County submitted and the voters approved in 2006 and again in 2011 the veterans and human services levy providing dedicated revenue for health and human services, and
      WHEREAS, in 2007, the metropolitan King County council authorized the mental illness and drug dependency sales tax, and
      WHEREAS, the King County adopted strategic plan identifies the principle of "fair and just" to guide the county's work and services in order to achieve equitable opportunities for all people and communities, and
      WHEREAS, despite the fact that King County has made progress in improving residents' health, significant and unacceptable disparities persist in relation to geography, race/ethnicity, and other social factors, and
      WHEREAS, the United States outspends every other industrialized nation on healthcare yet lags behind its peers on health status, and
      WHEREAS, in 2010, the United States Congress passed the affordable care act which aims to expand coverage, improve quality and control costs through numerous care delivery, payment and public health reform provisions, and
      WHEREAS, the conditions in which county residents live, learn, work and play have a greater impact on their health and life expectancy than do medical conditions and health care services, and
      WHEREAS, improving health and human services outcomes for county residents requires collaboration among partners, providers and policymakers, and
      WHEREAS, it is imperative to create and implement innovative health and human service models that integrate services across numerous sectors such as housing and homelessness, behavioral health services, clinical health services, community prevention, public health, businesses and others, and
      WHEREAS, in 2011, the executive convened a health care reform planning team to serve as a forum to design models of integrated service delivery that are based on a thorough understanding of broader community strategies that will produce the best possible health and human potential outcomes, and the health care reform planning team developed a framework for an accountable and integrated system of care for low-income residents in the county;
      NOW, THEREFORE, BE IT MOVED by the Council of King County:
      A.  The executive is requested to, in collaboration with the departments of public health and community and human services and a community stakeholder panel, develop and submit for council review and approval, a plan for an accountable and integrated system of health, human services, and community-based prevention for the county's residents in need.  The plan should include and build upon existing work including, but should not be limited to, the framework for an accountable and integrated system of care for low-income residents in King County.
      B.  The executive is requested to develop the plan with input from representatives from the health and human services community, the prevention and public health community, the business community, the geographic subregions of the county and local and national experts in system transformation and innovation, and with assistance from council staff.
      C.  The plan should specifically address the systems of health, human services and community-based prevention that are operated and funded by King County, and should describe coordination and integration strategies across a variety of health and human services including, but not limited to, the following service areas:
        1.  Clinical health services, including primary care;
        2.  Health promotion and community-based prevention;
        3.  Community development and place-based programs;
        4.  Housing and homelessness;
        5.  Mental illness and chemical dependency, including recovery oriented systems;
        6.  Domestic violence and sexual assault survivor; and
        7.  Employment and job training; and
        7.  Developmental disabilities.
      D.  The development of the plan should recognize the various populations and diversity of those in need services throughout the county, including, but not limited to, infants, children and youth, families, seniors, veterans and culturally specific populations.
      E.  The plan should be individual, family and community centered, and should provide for culturally appropriate, evidence-based or evidence-informed strategies, services and programs that are cost-effective, integrated, prevention focused and recovery oriented.
      F.  Among the policy goals that the plan should address are:
        1.  The simultaneous pursuit of an improved experience of health care and human services for residents;
        2.  Improved health of the residents;
        3.  Lowered or controlled costs; and
        4.  Options for the creation and implementation of a single point of accountability for health and human services cost, quality and outcomes.
      G.  The plan should include, but not be limited to, the following:
        1.  A section that describes the vision and goals of an accountable and integrated system of health, human services and community-based prevention that specifically integrate with existing county policy;
        2.  A section describing the implementation of an accountable and integrated system of health, human services and community-based prevention;
        3.  A section detailing how an accountable and integrated system of health, human services and community-based prevention will coordinate with existing King County work and planning processes such as the healthcare reform planning team, the community link project, the committee to end homelessness, the veterans and human services levy citizen oversight boards and the mental illness and drug dependency oversight committee;
        4.  A section that addresses the role of performance measurement and includes an outline of the potential metrics for assessing progress and success in implementing and refining an accountable and integrated system of health and human services and community-based prevention;
        5.  Recommendations for strategic investments to achieve an accountable and integrated system of health, human services, and community-based prevention that reflect:
          a.  advancement of the King County Strategic Plan and equity and social justice initiative;
          b.  integration of models that demonstrate the ability to enhance health and human service outcomes for the county's most-vulnerable residents;
          c.  integration of innovative strategies that are specifically applicable in under-resourced communities experiencing significant inequities;
          d.  the opportunities and efficiencies of building human service and prevention partnerships in tandem with the health delivery system for low-income residents; and
          e.  improvements in system capacity, quality, effectiveness and efficiency of the county's health, human services and community-based prevention systems; and
        6.  Options for financing accountable and integrated system of health, human services, and community-based prevention in the county by using existing or new revenue generating authority such as allowed under RCW 82.14.450.
      H.  The plan should be submitted to the council, with a motion for acceptance of the plan, by June 1, 2013.
