A RESOLUTION approving the scope of project changes for the Healthcare for the Homeless Grant.
Body
      WHEREAS, the Board of Health is the formal governance board for the Public Health-Seattle & King County Health Care for the Homeless grant under section 330(h) of the Public Health Services Act, and
      WHEREAS, the Board of Health is responsible for reviewing and approving what are known as "Change in Scope of Project" requests and documenting these approvals in the Board minutes, and
      WHEREAS, the attached Scope of Project Tracking Document lists current Health Care for the Homeless project sites as well as proposed changes in scope of project;
      NOW, THEREFORE, BE IT RESOLVED by the Board of Health of King County:
      The Board approves the following Scope of Project changes:
      A.  Mid-2012, White Center Public Health Center will move to a new location, 9930 -8th Avenue SW, Seattle, WA 98106.  The name of the new facility is White Center Public Health Center at Greenbridge; and
      B.  Public Health - Seattle & King County will establish a new primary care practice in mid-2012 colocated with Navos Mental Health Solutions at 1010 S. 146th Street, Burien, WA 98168, with plans to move later in 2012 with NAVOS adult client services to a new Navos facility in Burien at 1210 SW 136th Street, Burien, WA 98166.
